In addition to this registration form, youth participants in the MIPA Summer Journalism Workshop may be required to submit permission forms, to be completed by a parent or guardian. MIPA will send these forms to the participant and the parent via the email addresses included on this registration form.

For school field trips to MIPA events, teachers maintain appropriate permission forms and medical authorization forms as required by the school.
